Total PSA is a parameter used to detect prostate disorders, including cancer. A PSA test is recommended from the age of 45, or earlier if there is a family history of prostate cancer. Knowing your levels is a first step towards early detection.

It is important to screen men according to age, history and PSA values as it is the most common cancer in men over 70 in Europe and causes 3% of deaths in men in the European Union. It is rare in men under 50 years of age and the mortality rate in men under 60 years of age is low. In Spain, around 80 cases/100,000 inhabitants are diagnosed annually, with 711 PT3T over 65 years of age. Screening is associated with a higher diagnosis of prostate cancer and an earlier stage at diagnosis without increasing overall survival, according to the latest studies.

It is now generally considered to be:

  • Normal PSA level: a total PSA concentration of less than 4ng/mL (nanograms per millilitre of blood).
  • High PSA level: values above 10.0 ng/mL are associated with an increased risk of prostate cancer (50%).
  • Values between 4.0 and 10.0mg/mL are non-specific and may be due to prostate cancer, benign prostatic hyperplasia or prostatitis:
  • The risk of developing prostate cancer is 6.6% for PSA <0.5ng/mL.
  • From 10% between 0.6-1ng/mL.
  • From 17% between 1.1 - 2ng/mL.
  • From 23% between 2.1- 3ng/mL.
  • From 26.9% between 2.1- 3ng/mL.

The prostate cancer prevention test does not require fasting. It is recommended to avoid strenuous exercise, cycling or exercises that put pressure on the pelvic area, to refrain from ejaculating 24 hours before the test, to wait several weeks after a rectal examination or a prostate biopsy, to avoid taking the test if you have a prostate infection (prostatitis), discontinue certain herbal products as there is evidence that they raise PSA and report if you are taking drugs that may affect PSA, such as 5-alpha-reductase inhibitors (Finasteride or Dutasteride), as these drugs may lower PSA levels and mask the results.   

Although PSA is a marker that fluctuates significantly on a continuous basis, its annual determination as a screening or preventive measure is the most appropriate. In the case of known benign or malignant prostate pathology, it should be measured every six months, with few exceptions.
